Attempts to collect a debt that was already paid, such as by your client, an insurer, a government payor like Medicare or Medicaid, or another party;

Attempts to collect a debt for a medical service that your client did not receive. This can include instances where your client was billed for more expensive or complex services than what was actually provided;

Attempts to collect an amount that exceeded a federal or state cap, such as the limits set by the No Surprises Act or another statute that limits a patient’s obligation to pay for a medical bill;

Attempts to collect a medical debt without sufficient documentation of the amount owed and whether any of it had been paid;
